Innovative Care Coordination (ICC) Team Meetings

In order to promote innovative care coordination projects and provide improved care to students with chronic diseases, especially those in high risk populations, the ESHS program plans to provide funding for school districts to implement projects that meet these objectives. This innovative project should include consideration of cost efficiencies and cost effectiveness in the delivery of this care. The project should include a team approach within the district supported by school administrators, special education teams, teachers and families to increase the understanding of what is required for care coordination in the school setting.
